Structured Support for Chronic Health

Managing a long-term health condition—such as persistent arthritis, Type 2 diabetes, or cardiovascular disease—can be physically and mentally exhausting. Medicare’s Chronic Disease Management (CDM) program (formerly known as Enhanced Primary Care or EPC) allows you to access subsidized allied health services to improve your quality of life.

Our university-qualified Exercise Physiologists work alongside your General Practitioner to implement a structured, evidence-based exercise strategy. By providing consistent, monitored movement, we help you reduce the severity of your symptoms, increase your physical resilience, and decrease your reliance on medication or hospital-based interventions.

Foundational Pillars of Our Medicare Services

  • Clinical Collaboration: We maintain open communication with your GP, ensuring your exercise program is medically appropriate and aligned with your broader health treatment plan.
  • Evidence-Based Management: Our interventions are backed by the latest clinical research for chronic disease, ensuring your program is highly effective for your specific condition.
  • Functional Longevity: We focus on teaching you physical strategies that help you manage your condition independently, allowing you to move with more ease and less pain.

Who Benefits Most from Medicare Programs?

We customize our mobile therapy plans to meet the clinical needs of clients managing long-term health issues, including:

  • Metabolic & Endocrine Conditions: Supporting clients with Type 2 diabetes or weight management challenges to improve insulin sensitivity and metabolic health.
  • Musculoskeletal Conditions: Providing relief and functional movement therapy for severe osteoarthritis, rheumatoid arthritis, and chronic back pain.
  • Cardiovascular & Pulmonary Issues: Designing safe, heart-rate-monitored programs for clients managing hypertension, high cholesterol, or COPD.
  • Osteoporosis & Bone Health: Focusing on safe, bone-strengthening resistance training to reduce fracture risk and improve structural density.

How We Deliver Your Medicare Program

  • Step 1: Obtain Your Referral: Speak to your GP about your health condition to see if you are eligible for a Chronic Disease Management (CDM) plan and a referral for Exercise Physiology.
  • Step 2: Initial Clinical Assessment: We perform a comprehensive evaluation of your medical history, symptoms, and functional barriers to build a baseline for your treatment.
  • Step 3: Supervised Mobile Sessions: We bring professional-grade equipment to your home or community setting, providing one-on-one coaching to ensure every exercise is performed safely.
  • Step 4: Progress Reporting: We document your improvements and provide feedback to your GP to ensure your treatment plan remains accurate and effective.
Frequently Asked Questions

You must visit your General Practitioner (GP) and request a Chronic Disease Management (CDM) plan. If your GP deems your condition is chronic and complex, they will provide you with a referral form specifically for Exercise Physiology.

Under a standard Chronic Disease Management (CDM) plan, Medicare provides a rebate for up to 5 allied health sessions per calendar year.

While Medicare covers a significant portion of the session cost, there may be a gap fee to cover our mobile service delivery. We provide transparent pricing information regarding gap fees during your initial booking.

Yes. We provide our Medicare-subsidized services as mobile visits, meaning we come to your home, which is ideal for clients whose chronic conditions make traveling to a clinic difficult.

Once your 5 sessions are complete, we will send a report to your GP outlining your progress. If ongoing therapy is medically necessary, your GP may review your condition and, if appropriate, issue a new referral for the following calendar year.
